Please follow the instructions pertaining to the set up of an RMA service.

1. The following information is required to setup an RMA service.

• Product Make     Example
• Product Model    Example
• Product Serial#  Example
• Purchasers Name
• Company Name (if applicable)
• Purchasers Address
• Date of Purchace
• Proof of Purchase

2. Once all the required information to set up an RMA service is confirmed please press Next and fill out our RMA form.
